Please review the following policies, as it will make your experience more smooth and comfortable, and it will help me to help you the very best way I can!  Also, please indicate at the bottom of this form that you have read and agree to abide by these policies.

Please note my office hours are Monday - Friday 8:30 AM – 4:30 PM Eastern Time.  If you call or email me or my team outside of those hours, we will promptly return your email/phone call on the next business day. 

All consulting calls/visits are by appointment only.  To ensure you achieve the very best results, please plan ahead and keep your appointments.  All appointments must be made at least 24 hours in advance. 

Canceling/Rescheduling appointments.  I want great results for you, so please try not to cancel our appointments.  If you need to reschedule, please give me at least 24 hours’ notice.  As with any professional business, a service fee of $25 will apply if less than a 24-hour notice is given for a cancellation. 

Action steps.  For our work together to have the desired impact (you feeling much better!), actions will be required on your part, such as following through on my food and lifestyle recommendations and reporting back on your progress.  If you merely call/show up for our appointments and fail to put my suggestions into place, you will not benefit from our work together. 

Confidentiality and Disclaimer.  As your nutritionist, I will maintain a highly confidential environment, and I will not, either directly or indirectly, use any information for my own benefit or that of Holistic Blends, Inc., nor will I disclose, or communicate, in any manner, any information to any third party unless legally required to do so.  In addition, the ideas, suggestions, and information I provide in our consulting sessions are not intended to take the place of, or be a substitute for, the care of a licensed physician or medical practitioner.  

Termination and Refund Policy. I am completely confident that you are going to benefit greatly from our working together. However, I understand that things happen and you may choose to terminate our consulting relationship. The following parameters apply if you have purchased a discounted package:

• Creating Your Digestive-Friendly Diet Plan: A $100 cancellation fee applies and will be deducted from all refunds.
• The Say Goodbye to Digestive Issues for Good Plan: A $150 cancellation fee applies and will be deducted from all refunds. 
• The Comprehensive Total Body Tune-Up Plan: A $250 cancellation fee applies and will be deducted from all refunds. 

The amount refunded will be the difference (if any) between the total charges for the unused portion of the package, less the applicable cancellation fee.  All termination and refund requests must be made in writing.

Insurance.  We do not participate in health insurance plans.  Statements of services rendered for submission to insurance or HSA plans will be provided upon request. 

My promise to you.  I promise to be there for you and give you 100% of my time, attention and expertise during our sessions.  My staff and I will always be kind and patient with you, so you know you always have a place to express your concerns without judgment.  I will do all I can to help you stay focused on your goals, and most of all, I care deeply about you and your success.
